Primary Care Services for the Homeless

Health Care for the Homeless is a Federally Qualified Health Center. We provide primary medical care and mental health services to individuals and families in Natrona County who are either homeless or at-risk of homelessness.

Our Services

All of our services are available to established patients only. If you are interested in establishing as a patient, please call us at (307) 235-6116.

Primary Health Care​

Chronic Disease Management

  • Team-based care appointments with your primary care provider and pharmacist
  • Care planning and management of chronic health conditions
  • Comprehensive patient education and medication management

Lab Services

  • Certified Phlebotomy Technicians
  • Routine labs and blood work
  • Testing for certain acute conditions
  • HIV and Hepatitis C testing

Mental Health Counseling

  • Collaborative treatment plans and goals created by clients and their counselor
  • Variety of evidence-based therapeutic modalities offered
  • Eye Movement Desensitization and Reprocessing (EMDR) Therapy

Medication Dispensary

  • Affordable prescription copays
  • Patient Assistance Programs
  • Medication education counseling by a licensed pharmacist

Who We Serve

Health Care for the Homeless can only serve those who are currently staying in Natrona County and who meet one or more of the following criteria: 

Those who are currently homeless or have been homeless within the previous 12 months, or who are at-risk of homelessness due to facing eviction or other circumstance

Those who do not have a stable place to stay and instead, temporarily stay in a series of other people’s homes

Those who are living in housing that poses a risk to the health, safety, or physical well-being of occupants, neighbors, or visitors

Those who are staying long or short term in a hotel/motel room

Those who are staying with another person temporarily without a lease or rental agreement

Those housed in transitional or rehabilitation housing programs like the Central Wyoming Counseling Center, Casper Re-Entry Center, Sober Living Houses, Safe Houses, or other transitional housing and rehabilitation centers

Those who are staying at the Central Wyoming Rescue Mission on any status

Those staying in unsheltered areas like vehicles, tents, parks, streets, or other public areas

Ways to Book An Appointment

Call Ahead

Call ahead to make your appointment. Same-day appointments are often available at our main clinic.

Walk-In

Walk-in appointments are available at the Central Wyoming Rescue Mission, 230 N. Park, Monday - Thursday from 7:00AM - 8:00AM.

Walk-in appointments are on a first-come, first served basis.

Telehealth

Telehealth appointments are available. Call to make your Telehealth appointment.


After Hours Assistance

If you are needing medical assistance after house, please call the office.

The answering service will contact the physician on call for help.

We are your patient-centered medical home.

The PCMH model adjusts how we deliver care to better serve patients and our community. It holds a promise of a new way to improve healthcare while fostering strong relationships with our patients.

Medication Dispensary

Health Care for the Homeless medication dispensary proudly dispenses medications at reduced costs for our patients. We are a small dispensary, with only one pharmacist and technician, but work hard to fill all of our patients’ needs.
