Health Care for the Homeless serves all patients regardless of ability to pay. Discounts for essential services are offered based on family size and income. Below are some common forms of proof of income:

  • One month pay stubs for all employed adults
  • Unemployment check stubs
  • Social Security award letter
  • Most recent year’s federal income tax return

FAQs

Health Care for the Homeless (HCH) offers a Sliding Fee Discount Program (SFDP) for those persons whose income is at or below 200% of the poverty level. You must fill out a “Sliding Scale” application and provide information regarding the number in your household and verification of your income which will allow HCH to accurately determine the fee you and your family will pay for each medical and mental health visit.

All new and returning Health Care for the Homeless patients are eligible to apply for the sliding fee scale.

Our sliding fee scale application is included in our intake process for all patients who are interested in applying.

The types of income used to determine eligibility for SFSD include, but are not limited to, the following:

  • Employment and cash earnings
  • Unemployment compensation
  • Worker’s Compensation
  • Social Security
  • Supplemental Security Income
  • Public assistance
  • Veterans’ payments
  • Survivor benefits
  • Pension
  • Retirement income
  • Rents
  • Royalties
  • Income from estates
  • Interest/Dividends
  • Trusts
  • Educational assistance (scholarships and grants used for living expenses)
  • Alimony
  • Child Support
  • Assistance from outside the household
  • Misc. sources

HCH allows patients the option to self-report their income, but if you have proof of income, we would ask that you bring that to your first appointment with us.

HCH can update patients’ sliding fee scale information at any time. Please call us or let our front desk know if your information changes.
