Things to Know

Patient FAQs

Health Care for the Homeless is only available to those homeless or at-risk of homelessness in Natrona County.
  • You must be homeless or at risk of homelessness to qualify as a patient.
  • You must be currently staying in Natrona County to qualify as a patient.
  • You do not need to have photo I.D. to qualify as a patient.

Health Care for the Homeless is a primary care clinic. If you already have a primary care provider, it is not recommended to see two.

You must call our clinic to schedule your first appointment. Your first appointment will be a medical appointment with a primary care provider. After this initial appointment, you will be eligible to use our other services as needed.

We will schedule you our soonest available appointment, but this can sometimes be 2-4 weeks out.

  • During this call, you will be asked to provide basic information like name, address, date of birth, SSN, and contact information. We use this
    information to add you to our system to schedule you.
  • You may also be asked to speak to our care manager prior to being
    scheduled.
  • You will need to arrive 1 HOUR EARLY to your first appointment in order to complete our intake information.
  • Call as soon as possible if you are unable to make it to your appointment. If you do not call and do not show up to your first
    appointment, then you will be inactivated in our system.
Health Care for the Homeless uses Phreesia to do electronic intakes. If you provided us with a valid email address and/or cell phone number, you will receive a text or email with instructions on completing intake. You may complete the intake at home on a personal device prior to your appointment, but all new patients must show up at least 30 minutes early to their first appointment regardless.

If you would rather complete the intake process at our clinic, we will provide you with a Phreesia Pad to complete your intake. Please arrive 1 HOUR EARLY to your scheduled appointment.

If you are unable to complete or uncomfortable with electronic intake, ask our front desk for paper forms.

Please bring past medical information, or information about other providers that you have seen so we can request records and current medication lists. If you have a photo ID, insurance card, or proof of income, please bring those as well.

Appointments can be scheduled by calling our clinic or by requesting an appointment through our Patient Portal.

Call Ahead

Call ahead to make your appointment. Same-day appointments are often available at our main clinic.

Walk-In

Walk-in appointments are available at the Central Wyoming Rescue Mission, 230 N. Park, Monday - Thursday from 7:00AM - 8:00AM.

Walk-in appointments are on a first-come, first served basis.

Telehealth

Telehealth appointments are available. Call to make your Telehealth appointment.


After Hours Assistance

If you are needing medical assistance after house, please call the office.

The answering service will contact the physician on call for help.
